If your program offers you sessions with a health coach or clinician, they may have access to your information during your sessions to inform the conversation.
If your program does not offer you sessions with a health coach or clinician, then you are the only one who can see your assessment.

Do I have an existing website account?

If you are participating in the Care Management program and have had your Welcome Call from a nurse, a login account has already been created for you. To log in, enter the username and password provided to you on your Welcome Call.
Do I need to create a website account?
If you have never logged in to this website before and are NOT participating in the Care Management program, then on the Sign In page, click Sign Up and enter the requested information to create your new account.
If you've received a letter inviting you to participate in the Care Management program, but have not received a Welcome Call from one of our nurses yet, then on the Sign In page, you should click Sign Up to create an account.
To create a new account, you should follow this procedure:

Click the Sign Up link.
On the Sign Up page, type your first and last name, birth date, postal/zip code and gender.
Membership number"Your Health Plan Provider policy number
First Name, Last Name"Your full name. (Note that the website login username is automatically created.)
Postal/Zip Code"The same postal/zip code known by your Health Plan Provider for your address
Gender" Male or Female
Click Next.
Confirm your membership status with your Health Plan Provider, and then click Next.
Click Create New Account. The information that you entered is sent for authentication.
What if I forgot my Username or Password?
Don't worry, you can easily recover your username or password.

Verify yourself as a user by typing your first and last name, username, birthdate, and zip code.

On the Reset Password page, enter and confirm a new (case-sensitive) password.
Why was my account locked and how do I unlock it?
If you entered the wrong password three times, your account is locked. Then you'll have to reset your password. If you lock your account, you will be directed to contact the Care Enhancement Centre for further assistance.
